This is a form for you who are interested in participating in our research. In the form, we ask you to answer relevant questions concerning your situation. We expect it to take about 5 minutes to complete the form. This is a first reconciliation between you and us.

By submitting the form, I agree that my data will be stored and used by relevant staff at the Center for Bionics and Pain Research. The information will only be used for the purpose of determining eligibility to participate in research conducted by the Center.
